This policy explains how Revive Pet Health (“Revive,” “we,” “us”) handles information when you use our website and services. It applies to visitors, prospective customers, and customers in the United States.

1. Information we collect

Information you provide.

  • Identifiers: name, email address, postal address, phone number, state of residence.
  • Account and billing: account credentials and payment information. Card details are processed by our payment processor; we do not store full card numbers.
  • Animal health information: your pet’s name, breed, age, weight, symptoms, diagnoses, medications, medical history, and any records you choose to share with us.
  • Communications: messages you send us, intake form responses, and consultation records.

Information collected automatically. Pages visited, referring source, device and browser type, approximate location derived from IP address, and interactions with the site, collected through cookies and similar technologies.

Information from third parties. Records from your primary veterinary practice, where you authorize their release, and information from our payment processor and pharmacy partner necessary to fulfill your order.

2. How we use information

  • To provide veterinary consultations and to prepare, dispense, and ship protocols
  • To communicate about appointments, orders, refills, and your pet’s care
  • To transmit prescriptions to our partner compounding pharmacy
  • To share protocol summaries with your primary veterinarian at your direction
  • To process payments and manage subscriptions
  • To operate, secure, and improve the website and services
  • To send service messages and, where permitted, marketing you can opt out of
  • To comply with legal, regulatory, and recordkeeping obligations

3. What we do not do

We do not sell your personal information, and we do not share it for cross-context behavioral advertising. We do not sell or rent information about your pet to third parties for their own marketing purposes.

4. How we share information

  • Your veterinarian and our clinical team, to provide care
  • Our partner compounding pharmacy, to fill and ship your prescription
  • Service providers operating under contract on our behalf: payment processing, scheduling, email and SMS delivery, hosting, customer support, and analytics
  • Your primary veterinarian, at your direction
  • Legal and safety, where required by law, to enforce our terms, or to protect the safety of an animal or person
  • Business transfers, in connection with a merger, acquisition, or sale of assets, subject to this policy

5. Veterinary records

Records generated in the course of veterinary care are maintained in accordance with applicable state veterinary practice acts and recordkeeping requirements. Veterinary records are not covered by HIPAA, which applies to human health information, but we handle them with equivalent care and restrict access to those who need it.

6. Communications and text messages

If you provide a mobile number, you consent to receive appointment, order, and care-related messages from us. Message and data rates may apply. Reply STOP to opt out of non-essential messages; essential messages regarding an active prescription may continue. You can unsubscribe from marketing email using the link in any marketing message.

7. Cookies and analytics

We use cookies and similar technologies for site functionality, to remember preferences, and for analytics that help us understand which content is useful. You can control cookies through your browser settings. Disabling them may affect site functionality. We honor Global Privacy Control (GPC) signals where required.

8. Data retention

We retain information for as long as needed to provide the services and for the periods required by law. Veterinary and prescription records are retained for the minimum period required by applicable state law, which is typically several years after the last treatment, even if you close your account. Billing records are retained as required for tax and accounting purposes.

9. Your choices and rights

Depending on where you live, you may have the right to:

  • Know what personal information we have collected and how we use and share it
  • Access or receive a portable copy of your personal information
  • Correct inaccurate personal information
  • Delete personal information, subject to legal retention requirements
  • Opt out of sale or sharing for targeted advertising (we do neither)
  • Limit the use of sensitive personal information
  • Not be discriminated against for exercising these rights

Residents of California, Colorado, Connecticut, Virginia, and other states with comprehensive privacy laws may exercise these rights by contacting us using the details below. We will verify your request and respond within the timeframes required by applicable law. You may use an authorized agent where the law permits.

Note that we may be required to retain certain veterinary and prescription records notwithstanding a deletion request.

Appeals. If we decline your request, you may appeal by replying to our decision. We will respond to appeals within the period required by your state’s law.

10. Security

We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit and access controls. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.

11. Children

The services are not directed to children under 13, and we do not knowingly collect their personal information. If you believe a child has provided us information, contact us and we will delete it.

Our site may link to third-party websites. This policy does not apply to them, and we are not responsible for their privacy practices.

We will post any changes here and update the date above. Material changes will be communicated to active customers directly.
